The Subscription Center allows registration to qualified players. Registered Players are permitted to purchase New York Lottery subscriptions and view transactions and history related to these purchases.

You authorize the New York Lottery to use and release certain personal information, including your name, hometown and any photograph, video or audio recording, in any medium (including but not limited to newspaper, television, radio and internet), for purposes of marketing, advertising or other purposes of the New York Lottery. If you do not provide all or any part of the personal information requested, your subscription may not be processed. The New York Lottery requires your personal information to comply with the Internal Revenue Code and Tax Law income reporting and tax withholding requirements and under the authority of Tax Law section 1604 to announce New York Lottery prize awards. The New York Lottery's Director of Prize Payments is the custodian of subscription and prize records and may be contacted by mail at POB 7533, Schenectady, New York 12301-7533 or by phone at (518) 388-3300.

Under penalties of perjury, I certify that (1) the number shown on this form is my correct taxpayer identification number (TIN), (2) I am not subject to backup withholding due to failure to report interest and dividend income, and (3) I am a U.S. person as defined by the I.R.S. (For federal tax purposes, a U.S. person includes but is not limited to: an individual who is a U.S. citizen or U.S. resident alien; a partnership, corporation, company, or association created or organized in the United States or under the laws of the United States; any estate (other than a foreign estate); or a domestic trust (as defined in 26 C.F.R. 301.7701-7)).
