Optional information you submit may be used in combination with certain mandatory information, such as name, physical address or email address for other purposes such as research and marketing or other New York Lottery purposes.   • Mandatory Non-Personal Information If you choose to purchase a New York Lottery subscription through our website, we will need to collect additional information from you, including the game you want to play and details about the type and frequency of that play.
Personal information notice
To process your subscription application, you authorize the New York Lottery to collect personal information about you. Under penalties of perjury, I certify that (1) the number shown on this form is my correct taxpayer identification number (TIN), (2) I am not subject to backup withholding due to failure to report interest and dividend income, and (3) I am a U.S. person as defined by the I.R.S.
